Person in charge during the initial inspection does not have a city of Chicago sanitation certificate. Priority foundation violation.

City of Chicago food service sanitation certificate

No city of Chicago certified food manager on site during the inspection while tcs foods were being prepared and served. Manager was telephoned upon my arrival and arrived on site approximately 15 to 20 minutes later. Priority foundation violation 7-38-012 citation issued.

Adequate handwashing sinks properly supplied and accessible

Noted the hot grill cooks line hand washing sink not accessible for use. Sink being blocked by a container of water inside the sink containing the cooks line utensils being used and stored in the sink. Reviewed the hand washing sinks are for hand washing purposes only. Priority foundation violation 7-38-030(c)

Food-contact surfaces: cleaned & sanitized

Observed a food handler on the cold sandwich prep cooler using a food encrusted dirty wiping cloth to wipe off the knife being used to cut ready to eat sandwiches. Then put the knife back on the prep surface to be used. Reviewed with the person in charge and the knife was removed. Priority foundation violation 7-38-005 citation issued.

Summary report displayed and visible to the public

Summary report given and posted on 7-6-26 report #2639380 was noted covered by other framed documents and not visible to the public. Reviewed the summary report must be posted in plain view of the establishment customers at all times. Today's summary posted. Priority foundation violation 7-42-010(b).

Insects, rodents, & animals not present

Basement exit door in poor repair with excessive rust and open 1/4 inch gap on the bottom threshold. Instructed to repair to be a tight fitting seal.

Toilet facilities: properly constructed, supplied, & cleaned

Must provide a covered waste receptacle for all coed washrooms.

Physical facilities installed, maintained & clean

All holes in walls, ceilings, missing floor drain covers in basement and hallway of stairwell to the basement, missing ceiling tiles in dish room must be sealed/replace.
